Ana Paula is a registered nurse with over 15 years of experience in the home health care field, and has a background working as a neuro, ortho and med-surg nurse.
As President and Founder of Holistic Home Group, her mission is create a safe and supportive environment, where her patients can feel truly cared for, and where their holistic wellbeing is prioritized above all else.
What we value in the services we offer:
Patients and family will be taught how to properly store, administer and keep track of medication.
Care givers are knowledgeable on wound cleaning, dressing, changes, and on recognizing signs of infection.
Clients will be educated on their chronic conditions and corresponding care instructions, empowering them to better manage their health.
Vital signs and comprehensive health status are monitored, and changes are reported to patient’s physician or healthcare team.
The Holistic team not only includes Certified Nursing Assistants and Home Health Aids, but also Registered Nurses who have additional training and knowledge needed to provide quality care.
Holistic team members pay special attention to care plans, understanding each patients’ needs.
Holistic team is specialized in many forms of dementia, including Alzheimer’s, and are thus able to assist seniors with specialized care for dementia.
24-hour care provided through the Holistic team of caregivers, covering rotating 12-hour shifts.
